Adding extra details to a case

In the Extra Details tab the user will be able to add additional information into the case report form. Case Identification can be used to record details for contacting the case. Additional case identification provides Mailing Address fields if different to the residential address recorded in the form. For more information on Case Transfer see the “Transferring cases” help section.

The User Defined Fields allow the user to designate an additional field that is not included in the EpiSurv case report form. These User Defined Fields can be extracted using the custom reports tool.
