Outbreak Form

On selecting an outbreak from the Outbreak List, the Outbreak Form will be displayed. The user will be able to view and/or edit the outbreak (depending on access granted). PHS users can view but not edit outbreaks that are associated with other PHSs (identifying information will be hidden). Non-PHS users can only view outbreaks in EpiSurv.

There is an Outbreak Summary in the side panel of the opened outbreak report form which displays key details of the outbreak.

A Form Section Navigation index is also found in the side panel. This allows a user to navigate directly to different sections of the OUTBREAK REPORT FORM.

Data entry onto the outbreak form is validated for correctness and for completeness of mandatory fields. A message will be displayed, then the user will be able to hover over the highlighted field to determine the issue. An outbreak form cannot be saved until all issues are resolved.

The user can find help on how to fill out specific sections of the outbreak form using the ? buttons. In addition, tooltips are automatically displayed on hovering over some fields.

Where applicable, a dropdown list will be available for free-text fields to ensure consistency and accuracy. The user will be able to partially enter a value then EpiSurv will display pre-existing list items. For specific lists, the user will also be able to add a new item to the dropdown list or edit an existing item.

The user will be able to generate a PDF copy of the outbreak report form using the PRINT button in the case report form.
