COVID-19: Additional case information

Select an option that best reflects how the case/infection was discovered.

⚠ Please note: Routine testing in Managed Isolation/Quarantine Facilities should be reported as Other (specify) with the description as “Quarantine Facility Surveillance Testing”. ‘Day of quarantine of positive test’ is required.**

Contact of a case: for cases that are discovered due to having contact with a known COVID-19 case. This includes if a case was tested because of potential exposure to COVID-19 in a common setting (e.g. hospital or rest home where another case was identified), the reason for discovery should be reported as “Contact of a case”. ‘Day of quarantine of positive test’ may be relevant.**

Ill seeking healthcare due to suspicion of COVID-19: for cases suspected of having COVID-19 upon seeking medical care OR for cases presenting at a GP for a swab due to suspicion of having COVID-19 (even if not unwell). ‘Day of quarantine of positive test’ may be relevant.**

Detected at point of entry: for cases that have COVID-19 symptoms upon arrival and are therefore tested outside of normal testing protocols. ‘Day of quarantine of positive test’ may be relevant.**

Repatriation: for cases discovered upon return to New Zealand on a repatriation flight. ‘Day of quarantine of positive test’ is required.**

Routine respiratory disease surveillance: for cases that are detected through the sentinel general practice influenza-like-illness (ILI) surveillance programme which is carried out each winter, or through the Severe Acute Respiratory Infection (SARI) surveillance undertaken at Middlemore and Auckland City Hospitals. Intermittent survey: for cases that are detected through an organised, time-limited programme of group testing of asymptomatic people, e.g. PHU testing of specific groups based on risk exposure or in areas with low testing rates.

Routine testing of border staff: for cases working at the border (e.g. in airports, seaports) identified due to routine testing.

Routine testing of managed isolation/quarantine facility staff: for cases working in a Managed Isolation or Quarantine facility identified due to routine testing.

Other (specify): If a suitable reason for detection does not appear in the list, please select ‘Other (specify)’ and provide a brief description of infection discovery. ‘Day of quarantine of positive test’ may be relevant.**

**‘Day of quarantine the positive sample was collected’ must be entered for all cases that were discovered to have COVID-19 while in a Managed Isolation or Quarantine facility. It is sufficient to record this field as a single numeric value (e.g. 1, 3, or 12).
