COVID-19: Additional Outcome Details

Health Status (mandatory for all cases)

  • Recovered – if the case has recovered from COVID-19 infection (cases must be recovered to be marked ‘historical’).
  • Not recovered – if the case is considered to have a current infection. This selection should be reviewed within 30 days.
  • Death – if the case died before recovering from COVID-19 (COVID-19 may not be the cause of death).
  • Lost to follow up – if unable to contact the case to close the investigation (please continue attempts to contact a case for a minimum of 30 days before selecting this health status option).
  • Unknown – if health status cannot be determined (e.g. evidence of recovery is ambiguous).
  • Other, specify – if none of the above options reflect the health outcome, please provide details in the text field. For example, a case may have left the country before their positive test result reached the PHU, so the case was never managed in New Zealand.

If case was hospitalised, date discharged from hospital - please use the date of death if a case dies before they are discharged from hospital.
